Form Completion Fee Policy Update
Dear Valued Patient,
Effective October 13, 2025, Family Allergy & Asthma has standardized a $10 per page fee for most medical forms across all our locations.
What forms require a fee?
With the exception of school-related forms which remain FREE of charge, most medical forms will be subject to the new fee, including:
- FMLA
- Insurance
- Work Documentation
- Camp Forms
- Physician Letters
Requirements
- You must have been seen within the past 12 months (scheduling alone does not qualify)
- Both your form request and full payment must be submitted at least 30 days before your due date.
- Full payment is required before any form can be released.
Why this change? Medical forms have become increasingly complex and time-consuming. This fee ensures we can provide accurate documentation while maintaining quality care during appointments.
How to Submit Your Form:
- Make sure you’ve had a visit within the last 12 months.
- Submit your form at least 30 days before it’s needed via email to triage@familyallergy.com, by fax, or in person at any office.
- Once received, your form will be uploaded to your account and routed to your provider’s team for completion.
- Standard turnaround time is 72 business hours once all requirements are met.

Please Note:
Form requests submitted with less than 30 days’ notice may not be processed in time to meet your deadline. Providers may also decline requests that fall outside their scope of care.
