We do not knowingly collect or maintain personal information from any person under the age of thirteen (13) years. We may say “no” to your request, but we’ll tell you why in writing within 60 days. You must make your request in writing, and you must provide a reason for the request. You have the right to request that we limit the use or sharing of your health information for treatment, payment, or healthcare operations. If you have paid in full for a specific item or service out of pocket, you can request that we do not share thatinformation with your health insurer for payment or operational purposes. We will comply with your request unless we are required by law to share the information.

  • Request confidential communications: You may ask us to contact you in a specific way (for example, home or office phone) or to send mail to a different address. We will say “yes” to all reasonable requests. You must make this request in writing, and you must tell us how or where you wish to be contacted.

We will include all the disclosures except for those about treatment, payment, or health care operations, or certain other disclosures (such as any you asked us to make).   • Choose someone to act for you: If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information. We will make sure the person has this authority and can act for you before we take any action.

You will not be penalized or retaliated against in any way for filing a complaint.

We reserve the right, in our sole discretion, to modify this Privacy Policy at any time without notice to you. We will post all modifications to the Privacy Policy on the Site, and they will become effective immediately upon being posted to the Site. Your continued use of the Site following the posting of such modifications constitutes your acceptance of them.
