We do not knowingly collect or maintain personal information from any person under the age of thirteen (13) years. If you have paid in full for a specific item or service out of pocket, you can request that we do not share thatinformation with your health insurer for payment or operational purposes. We will comply with your request unless we are required by law to share the information.

  • Request confidential communications: You may ask us to contact you in a specific way (for example, home or office phone) or to send mail to a different address. We will say “yes” to all reasonable requests. You must make this request in writing, and you must tell us how or where you wish to be contacted.

We will make sure the person has this authority and can act for you before we take any action.
