Louisville Allergy & Asthma has Joined Family Allergy & Asthma

Effective as of November 1, 2018, Dr. David Pallares has joined Family Allergy & Asthma.

With additional locations and resources, Dr. Pallares will be continuing to care for his patients. As a result of this change, both the Louisville Allergy & Asthma Main office on Churchman Avenue and the Clarksville office have moved to Family Allergy locations that are just minutes away. The Audubon Plaza office is closed permanently.

If you have an appointment after October 31, Family Allergy’s staff will be reaching out to reschedule your appointment on his new schedule.

If you are on allergy shots, we have moved your vials to the new offices. Chuchman Avenue patients may resume their shots at Family Allergy’s South End office on Bluegrass Avenue beginning November 5. Clarksville patients may resume their shots at Family Allergy’s office on Veterans Parkway beginning November 7.
