RE: Important Update Regarding Network Participation with Anthem Medicaid Indiana

We hope this letter finds you in good health. We would like to inform you of an important change regarding our network participation with Anthem Medicaid. As of September 30th, Family Allergy & Asthma will no longer be in network with Anthem Medicaid. We understand that your healthcare coverage is essential, and we want to provide you with the necessary information to ensure a smooth transition of your care.
We recognize that this change may prompt questions or concerns, and we want to assure you that we are here to assist you throughout this transition process. We recommend that you take the following steps to ensure uninterrupted access to healthcare services:
- Contact Anthem Medicaid.
Reach out to Anthem Medicaid using the contact information provided on your insurance card. They will be able to guide you through the process of selecting a new in-network healthcare provider. - Select a New Provider.
Research and select a new healthcare provider who is in-network with Anthem Medicaid. This will help you continue to receive quality care with the least disruption. If you need assistance finding a suitable provider, please don’t hesitate to contact our office, and we will be happy to provide recommendations. - Medical Records Transfer
If you decide to transition to a new provider, we can assist in transferring your medical records to your new chosen provider. This will ensure your new healthcare team has access to your complete medical history. - Prescription Medications
If you are currently receiving prescription medications, please consult with your new healthcare provider to ensure a seamless continuation of your medication regimen.
